Abstract
Early diagnosis of autism is crucial for timely intervention, yet existing screening tools are often limited by administration time in clinical settings. This study developed and evaluated the Screening Tool for Autism in Two-Year-Olds-Brief Form (STAT-BF) for detecting autism in toddlers aged less than 36 months. In Study 1, two independent samples of 40 toddlers each (20 autistic and 20 with developmental delays [DDs]) were analyzed using chi-square tests to identify discriminative items. Five items (Turn-taking, Bubbles, Snack, Balloon, and Bag of Toys) were retained, with a cutoff score of 3 yielding optimal sensitivity and specificity. Study 2's validation, based on a sample of 251 toddlers (81 autism, 55 mild- autism, 115 DDs), yielded high sensitivity for autism (0.91; 74/81) and mild-autism (0.91; 50/55), together with good specificity (0.81; 93/115). Administration time was reduced to 5 to 10 min while maintaining screening accuracy. These findings suggest that the STAT-BF may serve as a feasible and efficient Level 2 screening tool for early autism detection among toddlers referred for developmental concerns, particularly in resource-limited clinical settings. Further validation in diverse populations and longitudinal follow-up is warranted to establish generalizability.Lay AbstractEarly support can make a big difference in the outcomes of autistic children, but they must first be identified as early as possible. Although reliable autism screening tools exist, many are too time-consuming for high-volume clinical settings. In this study, we developed and validated a shorter version of the Screening Tool for Autism in Two-Year-Olds (STAT) that takes 5 to 10 min. This brief tool focuses on five play-based activities that assess social communication skills in toddlers aged 18 to 36 months. We conducted a two-phase study in Taiwan. The first phase identified the most discriminating items, and the validation phase included 251 toddlers with autism, mild-autism, and developmental delays (DDs). Results showed that the brief form correctly identified over 90% of autistic children, including those with milder symptoms, and correctly ruled out autism in about 80% of children with DDs. This efficient screening method could help more health care providers detect autism early, especially in high-volume clinical settings with limited resources. Because the brief form uses structured play instead of parent questionnaires, it offers direct behavioral observation and increases cultural accessibility for families from a broad range of backgrounds. Autism spectrum disorder (ASD) prevalence is increasing globally, including in low- and middle-income settings. Early screening is essential; however, screening tools require local validation. The Screening Tool for Autism in Toddlers and Young Children (STAT) has been incorporated into Vietnam’s Ministry of Health guidelines, yet its diagnostic performance has not been formally evaluated. This cross-sectional study validated the STAT among 86 children aged 24–36 months with language delay attending a tertiary pediatric hospital in Ho Chi Minh City between December 2022 and August 2023. STAT results were compared with DSM-5-based clinical diagnoses, the diagnostic reference standard. The STAT was administered by certified speech therapists, and STAT results were not disclosed to the child psychologist conducting the DSM-5-based diagnostic evaluation. Diagnostic accuracy indices and receiver operating characteristic (ROC) analysis were performed to evaluate diagnostic performance and explore alternative cut-off values. The findings showed a predominance of male children (65.1%) and a high prevalence of ASD (74.4%). At the recommended cut-off of ≥ 2, STAT demonstrated high sensitivity (89.1%) but moderate specificity (50.0%), with positive and negative predictive values of 83.8% and 61.1%, respectively. The area under the ROC curve was 0.7656 (95% CI: 0.639–0.883), indicating fair overall discriminative ability. In an exploratory analysis, a data-derived cut-off of 2.5 yielded the highest Youden Index, with sensitivity of 73.4% and specificity of 68.2%. The exploratory 2.5 threshold requires independent validation before any change to the recommended ≥ 2 threshold is considered. Overall, these findings provide preliminary evidence supporting the existing STAT cut-off of ≥ 2 in similar high-risk clinical populations, while larger multicenter and community-based studies are needed to confirm the performance and generalizability of this cut-off. Preliminary evidence indicates that the Checklist for Chinese Toddlers with Autism exhibits sound psychometric properties and may serve as an early screening tool for Chinese children with ASD aged 12–24 months, however, external validation is still required to establish its generalizability.

Autism screening aids in early detection and diagnosis. Several screeners are available for toddlers 16 months and older, but there is little data on the accuracy of screening measures used in the first year of life. A community sample (N = 1641) completed three screeners-M-CHAT-R, ITC, VIRSA-online at 6, 9, 12, 18, and 24 months of age (including exploratory use of M-CHAT-R prior to 16 months). At 36 months, outcomes of ASD or Non-ASD were determined by tele-evaluation and outcome surveys. Psychometric properties were assessed for all screeners at all ages, both used individually and in combination. When used in the first year of life (6-12 months), psychometric properties were in the poor range, with area under the curve (AUC) < 0.70 for all three screeners, whether used in isolation or combination. At 18 and 24 months, the M-CHAT-R and ITC outperformed the VIRSA, with psychometric properties in the fair range (AUC 0.70-0.79) used in isolation and the good range (AUC ≥ 0.80) when used in combination. Using the M-CHAT-R and ITC at more than one age in the second year also fell in the good range. This study, using procedures to address limitations of prior screening studies and follow screen-negative cases, found that none of the three screeners had adequate psychometric properties in the first year. At 18 and 24 months, findings supported the use of multiple screeners, either in combination or longitudinally, for best prediction of autism diagnosis.
