TL;DR
The clinical spectrum of recessive CRX -associated retinal disease is expanded by identifying an EORD phenotype caused by a homozygous CRX variant, which underscores the functional importance of the homeobox domain and highlight the need for further studies to clarify genotype–phenotype correlations.
Abstract
To characterize the clinical and genetic findings in three affected siblings from a consanguineous family with a novel homozygous
CRX
variant and an early-onset retinal dystrophy (EORD) phenotype, and to review previously reported cases of recessive
CRX
-associated retinal disorders.
Comprehensive ophthalmic examinations, including visual acuity testing, fundus photography, optical coherence tomography, and full-field electroretinography when available, were performed in all participants. Whole-exome sequencing (WES) was carried out on the affected individuals, followed by variant validation and familial segregation analysis. Previously reported biallelic
CRX
variants and their associated phenotypes were compiled from the literature for comparative analysis.
WES identified a novel homozygous missense variant in
CRX
(NM_000554.6:c.158 A > G; p.Glu53Gly) located within the homeobox domain. Segregation analysis in this consanguineous family confirmed homozygosity in all three affected siblings and heterozygosity in two unaffected siblings, consistent with autosomal recessive inheritance. Affected individuals exhibited an EORD phenotype characterized by early visual decline, macular atrophy, peripheral pigmentary changes, and severe outer retinal atrophy. Review of previously reported cases revealed that recessive
CRX
variants are extremely rare, with most affected individuals presenting with Leber congenital amaurosis or retinitis pigmentosa phenotypes.
This study expands the clinical spectrum of recessive
CRX
-associated retinal disease by identifying an EORD phenotype caused by a homozygous
CRX
variant. These findings underscore the functional importance of the homeobox domain and highlight the need for further studies to clarify genotype–phenotype correlations.
Pathogenic variants in the
INPP5E
gene are associated with a broad phenotypic spectrum, from syndromic ciliopathies like Joubert syndrome to non-syndromic inherited retinal degenerations (IRD). Isolated retinal phenotype without systemic involvement remains rarely characterized. We describe a genetically conf...
CRXP197Rfs is a dominant-negative allele that produces cone-rod dystrophy with variable intrafamilial phenotypes that caution against genotype-based prognostication and support mutant-allele knockdown with gene augmentation as therapeutic strategies.

The findings suggest that this PCARE-associated inherited retinal diseases variant may be enriched in the Portuguese population, and further studies are needed to confirm the founder effect hypothesis and to guide future therapeutic strategies targeting PCARE-associated retinal degeneration.

Biallelic variants in the RDH11 gene, a retinol dehydrogenase involved in the visual cycle and systemic retinoid homeostasis, were initially implicated in a rare condition characterized by retinal dystrophy, early-onset cataract, neurodevelopmental anomalies and myopathy, through single-family reports, with this associ...

BACKGROUND
PRPF31-associated inherited retinal dystrophy is an autosomal dominant form of retinitis pigmentosa (RP), typically caused by haploinsufficiency. Although affected individuals generally exhibit early-onset nyctalopia and progressive visual field loss, the clinical and genetic characteristics associated with...

PURPOSE
We report the detailed ophthalmological evaluation of two affected members of the same family (mother and son) carrying the heterozygous pathogenic CRX frameshift variant (NM_000554.6:c.661del; p. Tyr221Thrfs *9), both presenting with photophobia and photopsia but exhibiting markedly different clinical manifest...
