Diagnostic Value of Ambulatory Blood Pressure Monitoring for Hypertension in Private Cardiology Practice in Senegal

Abstract

Hypertension is a major contributor to cardiovascular morbidity and mortality. Ambulatory blood pressure monitoring (ABPM) complements office blood pressure measurement by detecting white-coat hypertension, masked hypertension, and nocturnal blood pressure abnormalities, and this study aimed to assess its diagnostic value in the management of hypertension in private cardiology practice in Senegal. A prospective, descriptive, and analytical study was conducted from July to September 2024 at Clinique Urgence Cardio in Dakar, including patients aged 18 years or older with a valid 24-hour ABPM recording. Clinical, anthropometric, and treatment data, together with cardiovascular risk factors, were collected; office and ambulatory blood pressure measurements were compared; and logistic regression was used to identify factors associated with discordance between the two methods. Of the 104 patients assessed, 97 had valid ABPM recordings. The mean age was 53.2 ± 14.1 years, and women predominated. Cardiovascular risk factors included abdominal obesity (77.3%), known hypertension (54.6%), physical inactivity (43.3%), dyslipidemia (37.1%), and diabetes (16.5%). Mean office blood pressure was 145.7/91.4 mmHg, compared with 130.5/80.2 mmHg over 24 hours, 134.4/82.4 mmHg during daytime, and 125/75 mmHg during nighttime. ABPM identified sustained hypertension in 53.61% of patients, white-coat hypertension in 18.56%, masked hypertension in 9.28%, and normotension in 18.56%, with overall agreement between office and ambulatory measurements of 72.2%. Among patients with elevated office blood pressure, 25.7% had white-coat hypertension, while 33.3% of those with normal office blood pressure had masked hypertension. Non-dipping and reverse-dipping patterns were observed in 45.4% and 19.6% of patients, respectively, and discordance was independently associated with alcohol consumption (OR = 5.08; p = 0.007). ABPM improves the diagnostic classification of hypertension and reveals a high prevalence of discordant phenotypes and nocturnal blood pressure abnormalities, and wider use of ABPM may help optimize patient management in this setting.

Blood Pressure Control Following Fixed-dose Combination Antihypertensive Therapy in a Private Nigerian Cardiology Setting: A Retrospective Descriptive Study

Background: Hypertension is a leading modifiable risk factor for cardiovascular morbidity and mortality, with a disproportionate burden in low- and middle-income countries, including Nigeria. Questions remain regarding real-world prescribing patterns, blood pressure responses, and follow-up control rates among patients receiving combination antihypertensive therapy in Nigerian private clinical settings. Aim: To evaluate antihypertensive prescribing patterns, blood pressure changes, control rates, and documented treatment-related adverse events among patients with hypertension managed at a private multispecialist hospital in Lagos, Nigeria. Methods: A retrospective observational study was conducted among 563 adults with hypertension who had baseline and second-visit blood pressure measurements; 286 also had third-visit measurements. Demographic characteristics, comorbidities, treatment regimens, and blood pressure outcomes were analysed. Paired-samples t-tests were used to compare mean systolic and diastolic blood pressures across visits, while independent-samples t-tests and chi-square tests were used to compare patients who completed and did not complete follow-up. A p-value <0.05 was considered statistically significant. Results: The cohort had a mean age of 60.23 ± 14.12 years and was predominantly male (57.73%). Significant reductions in systolic blood pressure were observed, with mean decreases of 21.09 mmHg and 28.53 mmHg at the second and third visits, respectively (p < 0.001). Among patients with available follow-up blood pressure measurements, mean systolic blood pressure decreased significantly from baseline to the second visit and from baseline to the third visit. Blood pressure control was achieved in 219/563 patients at the second visit and 172/286 patients at the third visit. Conclusion: Antihypertensive therapy, particularly commonly used combination regimens, was associated with significant reductions in blood pressure and improved control rates across follow-up visits in this private Nigerian clinical setting. Prospective studies are needed to confirm the effectiveness, adherence benefits, and safety of fixed-dose combination therapy in broader Nigerian populations.

Prevalence and factors associated with controlled blood pressure among hypertensive patients in a Malaysian primary health clinic-a cross-sectional study.

BACKGROUND Controlled Blood pressure (BP) in hypertensive patients is essential in reducing the risk of cardiovascular diseases. Even a slight reduction in BP control may significantly improve cardiovascular outcomes. Several factors have been associated with controlled BP. We aimed to determine the prevalence of controlled BP among hypertensive patients and its associated factors in a Malaysian primary health clinic. METHOD A cross-sectional study was conducted among Malaysian adults aged 18 years and above with underlying hypertension who attended a public healthcare clinic located in a suburban area of Melaka, Malaysia, from December 2024 to February 2025. Participants were selected via systematic random sampling. Data were collected through self-administered questionnaires, medical records, and clinical blood pressure measurement. Blood pressure control status was determined based on clinic blood pressure readings measured during visits. Documented home blood pressure monitoring (HBPM) records, when available in the medical records, were reviewed as supplementary clinical information. A multiple logistic regression was conducted to determine the factors associated with controlled blood pressure. RESULTS A total of 460 hypertensive patients were included in this study, with a response rate of 96.8%. Only 41.5% of the participants (n = 191) had controlled BP. The median age was 64 (IQR:17) and the majority were female (57.4%, n = 264). The significant factors associated with controlled blood pressure among the study population were increasing age (aOR = 1.037, 95% CI [1.018,1.057], p < 0.001), female gender (aOR = 2.092, 95% CI [1.388,3.151], p < 0.001), participants who took one or two anti-hypertensive (aOR = 1.921, 95% CI [1.145,3.224],p = 0.013) and (aOR = 2.830, 95% CI [1.637,4.895], p < 0.001) respectively, participants without diabetes (aOR = 1.689, 95% CI [1.112,2.566], p = 0.014), and participant who had documented HBPM (aOR = 4.489, 95% CI [2.337,8.623], p < 0.001). CONCLUSIONS The findings suggest that the prevalence of controlled blood pressure, 41.5% is still low among the study population. Controlled blood pressure was associated with increasing age, female gender, patients on fewer than three antihypertensive medications, hypertensive patients without diabetes mellitus, and those who have documented home blood pressure monitoring.

Frequency of Retinopathy in Diagnosed Patients of Hypertension

Background: By definition, systolic blood pressure ?140 mmHg and/or diastolic blood pressure ?90 mmHg, measured on at least 2 separate occasions in a seated position during outpatient visits, irrespective of antihypertensive therapy, constitute hypertension. Persistent hypertension is associated with microvascular complications, including hypertensive retinopathy. Objective:  To determine the frequency of hypertensive retinopathy among diagnosed hypertensive patients presenting to a tertiary care hospital in Lahore. Methods: We conducted a cross-sectional descriptive study in the Medical Outpatient Department (OPD) of Fatima Memorial Hospital, Lahore. A total of 260 diagnosed hypertensive patients were included. All participants underwent clinical evaluation, including fundoscopic examination, to assess for hypertensive retinopathy. Results: Out of 260 patients, hypertensive retinopathy was identified in 189 (72.7%) patients, while 71 (27.3%) showed no evidence of retinal changes. Conclusion: A high frequency of hypertensive retinopathy was observed among patients with hypertension. Routine dilated fundoscopic examination is recommended to detect early retinal changes, which may reduce morbidity and prevent systemic complications.
