Splice Variants in ACY1 Associated with Congenital Hearing Loss: A Systematic Review of Pathology and Downstream Mechanisms

Abstract

Aminoacylase 1 (ACY1) deficiency is a rare autosomal recessive inborn error of metabolism traditionally associated with neurological manifestations and characteristic urinary N-acetyl amino acid excretion. The association between ACY1 variants and congenital hearing loss has remained incompletely characterized, with sensorineural hearing impairment documented in a minority of reported cases. This systematic review synthesizes evidence from 2000 to 2026 on splice variants in ACY1 and their relationship to congenital hearing loss, with particular focus on the recently characterized c.1063-1G>A splice-site variant and its downstream pathogenic mechanisms. We examine clinical, biochemical, and functional evidence from patient studies, in vitro splicing assays, and zebrafish models that collectively support ACY1 as a candidate gene for hereditary hearing impairment. Transcriptomic profiling reveals that ACY1 deficiency disrupts critical inner ear developmental genes, including gfi1ab and atoh1a/b, through mechanisms potentially involving aberrant BMP signalling pathway regulation. This review consolidates current knowledge, identifies knowledge gaps, and proposes a framework for understanding the molecular pathology of ACY1-related hearing loss. Clinical Presentation: The proband presented with bilateral sensorineural hearing loss of congenital onset. Notably, developmental milestones were normal over a 4-year follow-up period, distinguishing this case from many previously reported ACY1D patients who exhibited neurological symptoms including intellectual disability and motor delay. This observation suggests that hearing loss may occur as an isolated manifestation of ACY1 dysfunction, without the broader neurological phenotype typically associated with the condition.

Biallelic ABCA13 Loss-of-Function Variants in a Child With Neurodevelopmental Delay: A Case Report.

ABCA13 encodes ATP-binding cassette subfamily A member 13, one of the largest members of the ABC transporter family. Rare ABCA13 variants have been reported in neuropsychiatric and neurodevelopmental phenotypes, including schizophrenia, bipolar disorder, autism spectrum disorder, intellectual disability, and developmental delay; however, the mode of inheritance remains uncertain, and most published cases have focused on heterozygous variants in the context of a possible dominant or susceptibility model. We report a 5-year-old boy with neurodevelopmental delay, skeletal foot deformities, nonspecific dysmorphic features, convergent strabismus, and behavioral abnormalities. Initial genome sequencing analysis was nondiagnostic. Reanalysis after 6 months identified two rare predicted loss-of-function variants in ABCA13 in trans: c.2510del, p.(Leu837TyrfsTer21), a frameshift variant, and c.12064C>T, p.(Arg4022Ter), a nonsense variant previously reported in a patient with unexplained intellectual disability. The identification of compound heterozygous predicted loss-of-function variants supports the possibility that biallelic disruption of ABCA13 may contribute to neurodevelopmental disease, whereas previously reported heterozygous variants may represent incompletely penetrant risk alleles, susceptibility factors, or candidate findings rather than fully penetrant dominant causes. This case expands the emerging clinical and genetic spectrum associated with ABCA13 and supports further evaluation of a recessive model in patients with intellectual disability and neurodevelopmental delay.

Novel truncating SF1 gene variant in a family with mild neurodevelopmental disorder: expanding the phenotype of SF1-related spliceosomopathy spectrum

The SF1 gene encodes the splicing factor 1, a part of the splicing machinery. Recently, loss-of-function (LoF) variants in the SF1 gene have been suggested as a molecular cause of a neurodevelopmental spliceosomopathy. We report a unique familial case of a novel truncating variant in a single Czech family presenting with mild neurodevelopmental disorders (NDD) and variable congenital abnormalities. The c.1764_1776del variant in the last exon of the SF1 gene (NM_004630.4) was identified through exome sequencing in three affected family members. Functional analyses at the transcriptional level confirmed that this alteration does not trigger nonsense-mediated decay (NMD), and aberrant transcripts with premature termination codons are preserved. This study expands the spectrum of pathogenic variants involved in the development of new SF1 -related spliceosomopathy and its diverse phenotypic effects. Our data emphasise the clinical benefits of comprehensive exome sequencing for uncovering new gene-disease connections.
