Effectiveness of game-based learning in mental health nursing education on self-directed learning and academic self-efficacy among nursing students.

Abstract

Background

Traditional lecture-based teaching in nursing education often leads to limited student engagement, poor knowledge retention, and low self-confidence, particularly in complex fields such as mental health nursing, where students must navigate abstract clinical presentations and develop interpersonal skills that cannot be reduced to algorithmic protocols. Game-based learning (GBL) has emerged as an interactive strategy to enhance motivation, critical thinking, self-directed learning (SDL), and academic self-efficacy. This study aimed to evaluate the effectiveness of GBL integrated into a mental health nursing internship on SDL and academic self-efficacy among undergraduate nursing students.

Methods

A quasi-experimental, two-group, pretest-posttest design with a one-month follow-up was employed. Forty-six sixth-semester nursing students from Abadan University of Medical Sciences (Iran) were enrolled and allocated to an intervention group (n = 23) or a control group (n = 23) based on the pre-existing scheduling of their clinical internship rotations. After accounting for attrition, the final analytical sample comprised 40 participants (intervention: n = 20; control: n = 20). The intervention group received GBL integrated with standard instruction, while the control group received conventional internship training only. The intervention involved a custom-designed board game with scenario-based challenges delivered over 10 sessions (40 h total). Outcomes were measured using the Self-Directed Learning Readiness Scale (SDLRS) and the College Academic Self-Efficacy Scale (CASES) at pretest, posttest, and one-month follow-up. Data were analyzed using repeated measures ANOVA with Bonferroni-corrected pairwise comparisons for within-group changes, independent t-tests for between-group comparisons, and Cohen's d for effect sizes. A sensitivity analysis for attrition bias was conducted using a pattern-mixture model under best-case, worst-case, and intermediate imputation scenarios.

Results

Groups were homogeneous at baseline (P > 0.05 for all demographic and outcome variables). In the intervention group, repeated measures ANOVA revealed significant main effects of time for both SDL (F(2,38) = 38.74, P < 0.001, partial η²=0.67) and academic self-efficacy (F(2,38) = 42.51, P < 0.001, partial η²=0.69). Post-intervention, the GBL group showed significant improvements in SDL (mean difference: 16.25 ± 8.03, P < 0.001) and academic self-efficacy (mean difference: 12.50 ± 7.14, P < 0.001), with large effect sizes (Cohen's d = 1.25 and 1.56, respectively). These gains were maintained at the one-month follow-up, with no significant decline from posttest to follow-up (SDL: P = 0.784; self-efficacy: P = 0.821). The control group exhibited no significant changes across any time points (P > 0.05). Between-group differences were significant at posttest (SDL: P = 0.037, d = 0.68; self-efficacy: P = 0.029, d = 0.72) and at follow-up (SDL: P = 0.043, d = 0.66; self-efficacy: P = 0.034, d = 0.70). Sensitivity analyses confirmed that the findings were robust to plausible patterns of missing data due to attrition, with the treatment effect remaining significant even under worst-case imputation scenarios.

Conclusion

GBL integrated into a mental health nursing internship significantly enhanced SDL and academic self-efficacy, with effects sustained at one-month follow-up. These findings support the integration of game-based strategies into nursing curricula as a student-centered approach to bridge theory-practice gaps and foster durable self-regulatory capacities. Further research with larger samples, multi-site designs, and extended follow-up periods of six to twelve months is recommended to confirm the generalizability and long-term durability of these effects.

Effect of traditional versus video-assisted problem-based learning on pediatric nursing students’ academic self-efficacy and self-directed learning: implications for pediatric patients care

High-quality pediatric care depends on well-prepared nursing students. Innovative educational strategies, such as problem-based learning (PBL), have gained recognition for their potential to enhance students’ self-efficacy and self-directed learning, which are essential for providing safe and effective care to children. This study aims to compare the effects of traditional PBL versus video-assisted PBL on pediatric nursing students’ academic self-efficacy and self-directed learning in order to explore their implications for pediatric patient care. An experimental research study was conducted at the Faculty of Nursing, Benha University. A systematic random sample of 150 pediatric nursing students was utilized. Participants were evaluated using a structured interviewing questionnaire for knowledge, the Academic Self-Efficacy Scale, and the Self-Directed Learning Readiness Scale. The study compared experimental groups (Traditional PBL and Video-assisted PBL) against a control group. Following the intervention, significant improvements were observed in total knowledge, academic self-efficacy, and self-directed learning readiness among both experimental groups compared with the control group (P < 0.001). The video-assisted PBL group achieved the highest post-intervention mean scores in total knowledge (77.30 ± 4.42), academic self-efficacy (72.16 ± 6.18), and self-directed learning readiness (118.38 ± 3.76), followed by the traditional PBL group (72.22 ± 6.96, 67.70 ± 7.78, and 113.84 ± 11.72, respectively), whereas the control group demonstrated the lowest scores (59.98 ± 12.89, 45.74 ± 5.88, and 84.86 ± 10.28, respectively). Post hoc analyses further confirmed that video-assisted PBL significantly outperformed traditional PBL and conventional teaching across all measured outcomes (P ≤ 0.001). This study provides evidence that both traditional and video-assisted PBL are effective in improving students’ learning outcomes. However, Video-assisted PBL is superior in enhancing knowledge, academic self-efficacy, and self-directed learning readiness, offering a more effective strategy for nursing education. The effects of simulation-based pediatric nursing laboratory education on nursing students' clinical competence, self-efficacy, and worry: A randomized controlled trial.

AIM The primary outcome of this study was pediatric nursing competency following simulation-based laboratory practice in the pediatric nursing course, while the secondary outcomes included clinical worry and comfort, medication self-efficacy and knowledge related to vital signs assessment. BACKGROUND Simulation-based education provides nursing students with structured clinical experience in a safe, controlled environment. The quality and effectiveness of education are crucial to training competent professionals with theoretical knowledge and professional skills in pediatric nursing. DESIGN A pre-test and post-test, randomized controlled trial. METHODS The study was conducted in the nursing department of a university in northern Turkey during the 2023-2024 academic year. A total of 120 third-year nursing students were randomly assigned to either the experimental group (n = 60) or the control group (n = 60). Students in the experimental group received a two-day (16-hour) simulation-based pediatric nursing laboratory program, while the control group received a traditional laboratory education of equal duration. RESULTS No statistically significant between-group differences were observed in scale scores at baseline, post-intervention, or 2-month follow-up assessments. However, both groups demonstrated statistically significant improvements over time in several outcomes (p < 0.05), although these improvements were not consistently greater in the experimental group. CONCLUSION Simulation-based pediatric nursing laboratory education may be associated with improved clinical competence, self-efficacy and clinical comfort and reduced worry among nursing students; however, the findings should be interpreted cautiously given the limited between-group differences.

Playing for Change: A New Approach to Social Determinants of Health Education.

BACKGROUND Nursing students often have limited opportunities to apply social determinants of health (SDOH) concepts to patient experiences and clinical reasoning. This study evaluated a facilitated, game-based learning intervention to reinforce person-centered, socially responsive care. METHOD A mixed-methods pretest-posttest design was used with junior-level undergraduate nursing students at a public university. Students participated in a structured board game during class. Matched pre/post surveys (n = 47) were analyzed using paired-samples t-tests, and open-ended reflections underwent content analysis. RESULTS Significant improvements were found in endorsement of socioeconomic considerations in health care delivery (p = .020) and incorporating patient priorities into care plans (p = .026). Qualitative themes reflected increased awareness of SDOH, holistic thinking, engagement, and critical reflection. CONCLUSION A brief, facilitated game-based intervention may strengthen application of SDOH concepts within person-centered care. Findings support continued refinement and further study with larger, multi-site, and longitudinal designs.
