TL;DR

This review synthesizes recent evidence on probiotics, including both viable and heat-inactivated strains of Lactobacillus and Bifidobacterium, as modulators of osteoporosis, and concludes that specific strains improve bone density and microstructure.

Abstract

Osteoporosis, a metabolic bone disorder affecting nearly one-third of women and one-fifth of men globally, poses a major public health burden. Current pharmacological treatments face limitations, driving interest in safer nutritional interventions. The gut-bone axis, through which gut microbiota modulate bone homeostasis, offers a promising target. This review synthesizes recent evidence on probiotics, including both viable and heat-inactivated strains of Lactobacillus and Bifidobacterium, as modulators of osteoporosis. Probiotics exert integrated protective effects by reinforcing intestinal barrier, balancing Th17/Treg/Breg immunity, regulating RANKL/RANK/OPG signaling, producing short-chain fatty acids, and modulating serotonin metabolism. Preclinical and clinical studies show that specific strains improve bone density and microstructure, though efficacy depends on strain, dosage, duration, and host factors. Moreover, heat-inactivated forms and postbiotics offer stability advantages. Future development of synbiotics, next-generation probiotics, and functional foods tailored to specific matrices, along with personalized strategies, promises effective dietary interventions for bone health.

Intestinal–Bone Axis Mediated by Bifidobacterium: Mechanistic Analysis and Therapeutic Potential in Osteoporosis

A four-tier mechanistic framework of Bifidobacterium-mediated regulation of osteoporosis is established in which the evidence is stratified into three categories—direct evidence from Bifidobacterium-specific studies, indirect evidence from other probiotics, and general mechanisms of gut microbiota-regulated bone metabo...

Research Progress on Mechanisms of Milk-Derived Functional Components in Regulating Bone Health and the Gut–Bone Axis

Osteoporosis affects over 200 million people worldwide, with postmenopausal women at the highest risk. The treatment benefits of currently available drugs are constrained by safety concerns, and they lack bidirectional regulation of bone metabolism, driving growing interest in safe dietary interventions. Milk is rich i...

Gut microbiota-immune-bone axis in osteoporosis: mechanisms and intervention strategies

Osteoporosis (OP) remains a major global health burden, yet current pharmacological interventions are constrained by adverse effects, limited efficacy, and a bone-centric therapeutic paradigm that overlooks upstream pathogenic drivers. Emerging evidence positions the gut microbiota (GM) as a critical rheostat of bone h...

Gut Microbiota-Derived Short-Chain Fatty Acids Mediate the Bone-Protective Effects of Metformin Against Postmenopausal Osteoporosis.

BACKGROUND Dysbiosis-induced inflammation has been implicated in postmenopausal osteoporosis (PMOP). Although metformin has shown benefits for bone health, the exact role of the gut microbiota in the mechanism underlying this relationship remains unclear. HYPOTHESIS/PURPOSE We hypothesised that metformin could allevi...
