EP33 - ECE_2563 - Revisiting the 10-HU threshold: unenhanced CT attenuation in functional adrenal adenomas

Aug 2026 · European Journal of Endocrinology · Vol 195 · 0 citations

TL;DR

Unenhanced CT attenuation >10 HU is common in cortisol-producing adenomas, observed in approximately two-thirds of patients, and is also frequently seen in PA and MACS, and should be carefully considered in routine clinical decision-making.

Abstract

Although adrenocortical adenomas typically show low unenhanced attenuation (≤10 HU) on CT, this benignity criterion has limited sensitivity, particularly in lipid-poor and hormonally active tumors. We aimed to evaluate unenhanced CT attenuation across different functional and non-functioning adrenal adenoma subtypes. In this retrospective study, 167 surgically proven patients with non-enhanced CT of the adrenals as well as complete clinical and biochemical records were included: unilateral primary aldosteronism (PA) defined by complete or partial biochemical success after adrenalectomy (n = 64), mild autonomous cortisol secretion (MACS) (n = 48), Cushing's syndrome (CS) (n = 31), and compared to non-functioning adrenal adenomas (NFA) (n = 24, no surgical proof). After testing for normality, attenuation values were compared using the Kruskal-Wallis test. Frequencies were compared using Fisher's exact test. Adrenal adenomas in CS showed higher attenuation (19.3 ± 14.0 HU) than PA (10.5 ± 12.7 HU), MACS (6.8 ± 16.0 HU), and NFA (5.4 ± 19.9 HU, P = .003). Notably, lesions with >10 HU were common not only in CS (67.7%) but also in PA (48.4%) and MACS (43.7%). Values >20 HU were significantly more common in CS than in the other groups (51.6%, P = .029). In CS, attenuation correlated positively with baseline cortisol (r = 0.508, P = .035), midnight salivary cortisol (r = 0.454, P = .013), and 1 mg DST (r = 0.611, P < .001). In multivariable analysis, cortisol (P = .014) and 1 mg DST (P = .015) independently predicted attenuation (R2 = 26.2%). Unenhanced CT attenuation >10 HU is common in cortisol-producing adenomas, observed in approximately two-thirds of patients, and is also frequently seen in PA and MACS. This finding should be carefully considered in routine clinical decision-making.

Spectral CT-derived Adrenal Extracellular Volume for Functional Lateralization in Primary Aldosteronism.

RATIONALE AND OBJECTIVES There is currently a lack of validated noninvasive method for subtyping patients with primary aldosteronism (PA). This study aimed to investigate the feasibility of using spectral CT-derived adrenal extracellular volume (ECV) map for determining functional lateralization in PA. METHODS This s...

CT-based radiomics analysis adds value to glycometabolism assessment in adrenal adenoma: an exploratory observational study.

OBJECTIVE This exploratory observational study examined CT radiomics features associated with glucose metabolism in adrenal adenoma (AA). METHODS A total of 114 patients were initially enrolled, 106 who completed continuous glucose monitoring (CGM) were analyzed and classified as impaired glucose metabolism (IGM; HbA...

Predicting Functional Status in Adrenal Adenomas: The Role of Radiological Findings

Aim: The increasing use of imaging modalities has led to a higher detection rate of adrenal incidentalomas. Surgical treatment is recommended for hormonally active (functional) lesions and for non-functional lesions larger than 4 cm due to the risk of malignancy. Determining the functional status of adrenal masses gene...

Diagnostic performance of 3T MRI preoperative localization of parathyroid adenomas in primary hyperparathyroidism

Objective Preoperative localization is essential for the surgical management of primary hyperparathyroidism (PHPT). Ultrasound and 99mTc-sestamibi SPECT/CT are first-line imaging modalities, while MRI’s role is less defined. This study aimed to evaluate MRI for parathyroid adenoma localization in PHPT and compare it wi...

Long-term radiologic monitoring remains mandatory in non-functioning pituitary adenomas especially in the presence of an adenoma remnant

Resection with transsphenoidal surgery (TSS) is the mainstay of therapy for symptomatic non-functioning pituitary adenomas (NFPA). It is currently unclear whether radiologic follow-up could be discontinued in selected patients with favorable medium-term post-operative course. We performed a retrospective longitudin...

AVS-referenced diagnostic performance of 68Ga-Pentixafor PET/CT for detecting unilateral disease in primary aldosteronism: a systematic review and meta-analysis

Background Differentiation of unilateral from bilateral primary aldosteronism (PA) is crucial for identifying patients who are likely to be cured by adrenalectomy. This review evaluated the adrenal vein sampling (AVS)-referenced performance of 68Ga-Pentixafor positron emission tomography/computed tomography (PET/CT) f...
