TL;DR

This study developed and demonstrated the clinical applicability of disease-specific, population-specific RCI and SRB indices for Traditional Chinese-speaking patients with TLE to provide an empirical standard for evaluating meaningful cognitive changes after epilepsy surgery.

Abstract

Purpose

This study aimed to develop reliable change index (RCI) and standardized regression-based (SRB) change scores for Chinese versions of the neuropsychological tests to provide an empirical standard for evaluating meaningful cognitive changes after epilepsy surgery for patients with temporal lobe epilepsy (TLE).

Methods

Thirty-one non-operated patients with TLE were assessed twice (6-month interval) by using the Wechsler Adult Intelligence Scale-III (WAIS-III) and the subtests of the Wechsler Memory Scale-III (WMS-III) to compute RCI with the correction of practice effect and SRB equations. The predictor variables for the SRB models included the demographic variables (age, gender, education), clinical variables (age of seizure onset, seizure duration), and baseline performance. These standards were then applied to sixty patients who underwent selective amygdalohippocampectomy (SAH).

Results

Test-retest reliability was acceptable while the individual subtest variability was noted (r = 0.51 to 0.94). Baseline performance was the primary predictor in each SRB equation, accounting for 31.4% to 88.4% of the variance. Among the SAH cohort, 80.0%-100.0% remained cognitively stable across different tests. However, the patients in left SAH group exhibited a significant decline in verbal memory (20.0-23.3%), working memory and executive control (6.7-30.0%). Conversely, more than 10% of the right SAH group showed deterioration in visual memory and processing speed.

Conclusion

This study developed and demonstrated the clinical applicability of disease-specific, population-specific RCI and SRB indices for Traditional Chinese-speaking patients with TLE. These indices provide a standardized framework for accounting for practice effects, regression to the mean, and test-retest variability when interpreting meaningful cognitive changes following epilepsy surgery.

Measurement properties of the multifactorial memory questionnaire for evaluating subjective memory in people with epilepsy.

As a comprehensive measure for evaluating subjective memory concerns in adults with epilepsy, the MMQ has demonstrated promising psychometric properties, however, future studies should clarify the dimensionality of the MMQ in this population and address measurement error, which could limit its utility in monitoring ind...

Neurobehavioral outcomes following epilepsy surgery in tuberous sclerosis complex: A retrospective study.

INTRODUCTION Tuberous sclerosis complex (TSC) is a rare genetic disorder with a high prevalence of drug-resistant epilepsy. In such cases, epilepsy surgery may render 60% of suitable candidates seizure-free. Although seizure reduction may contribute to improvements in cognitive or language skills, little is known about...

Reconsidering the clinical utility of the Test of Premorbid Functioning (TOPF) as a measure of premorbid cognitive capability in patients with temporal lobe epilepsy.

Findings suggest that the TOPF is susceptible to TLE pathology, which violates the assumptions that underpin the use of irregular word reading as an estimate of premorbid cognitive capability in this population.

Validation and interpretation of the Persian Multiple Sclerosis Neuropsychological Screening Questionnaire.

BACKGROUND Cognitive impairment affects between 50 and 60% of people with multiple sclerosis (PwMS). Cognitive impairment usually includes problems with memory, speed of processing information, learning, and executive functions. This can impact everyday activities and quality of life. Cognitive Outcomes During Adjunctive Cenobamate Treatment in Patients with Drug-Resistant Epilepsy: An Interim Analysis of a Prospective Observational Study.

Moderate cognitive deterioration was observed following adjunctive cenobamate treatment in a substantial proportion of patients, particularly in attention, verbal memory, processing speed, and executive functions, in contrast, a subset of patients showed improvements, primarily in visual memory and phonemic fluency.
