Abstract

Introduction. Implantable cardioverter-defibrillators (ICDs) have been shown to reduce mortality in patients with both ischemic and non-ischemic cardiomyopathy (NICMP) by effectively terminating life-threatening ventricular arrhythmias. However, the distribution of ventricular arrhythmias varies across patient subgroups depending on the indication for implantation and the underlying etiology. Purpose. To evaluate clinical and electrocardiographic predictors of appropriate ICD therapies in secondary prevention patients with ischemic and non-ischemic cardiomyopathy. Materials and methods. A retrospective cohort study was conducted involving 58 patients who underwent ICD implantation for secondary prevention, including 37 with ischemic cardiomyopathy (ICMP) and 21 with NICMP. Baseline and follow-up assessments included transthoracic echocardiography, electrocardiography (ECG), lipid profile, and measurements of blood urea and serum creatinine. Results. The mean age of patients with NICMP was 55±9 years. Multivariate analysis identified fragmented QRS complex (fQRS), reduced LVEF (≤35%), and a wide QRS complex as independent predictors of appropriate ICD therapy. Conclusion. In patients with heart failure receiving ICDs for secondary prevention, the presence of fQRS, low LVEF, and wide QRS duration were independent predictors of appropriate ICD interventions. These parameters may serve as valuable tools for risk stratification, helping to identify patients who are most likely to benefit from ICD implantation and optimize healthcare resource utilization.

Predictive Value of the Electrical Risk Score for Ventricular Arrhythmias in Heart Failure Patients with Primary Prevention Implantable Cardioverter Defibrillators

Background: Implantable cardioverter defibrillators (ICDs) are essential for the primary prevention of sudden cardiac death in heart failure (HF); however, appropriate shocks increase mortality and impair quality of life. The electrical risk score (ERS) is an electrocardiographic (ECG)-based composite index with proven...

International Experience With Implantable Cardioverter Defibrillators for the Prevention of Sudden Death in High-Risk Patients With Hypertrophic Cardiomyopathy.

In this international multicenter study, the largest to date, including >3300 consecutive patients with HCM and ICDs, device therapy terminated potentially lethal ventricular tachyarrhythmias in 1 of 6 patients, with low HCM-related mortality rates for patients with ICDs.

The impact of inappropriate shocks in implantable cardioverter-defibrillators in contemporary clinical practice

Implantable cardioverter-defibrillators (ICDs) and cardiac resynchronization therapy defibrillators (CRT-Ds) are effective in preventing sudden cardiac death. However, inappropriate shocks (Inap-S) may impair quality of life and have been associated with adverse clinical outcomes. This study evaluated the incidence...

All-cause mortality and appropriate implantable cardioverter-defibrillator shocks with first-line catheter ablation versus antiarrhythmic drugs in ischemic ventricular tachycardia: A systematic review and Meta-analysis of randomized controlled trials.

In antiarrhythmic-naïve patients with ischemic cardiomyopathy and VT, first-line catheter ablation reduced appropriate ICD shocks but did not demonstrate a statistically significant reduction in all-cause mortality.
