Abstract

Continuous professional development (CPD) is a statutory and ethical requirement in contemporary medical practice, intended to sustain competence, professional accountability and patient safety. This review critically evaluates CPD in medical education by examining its theoretical foundations, regulatory importance, pedagogical models, benefits, innovations and limitations. CPD has evolved from didactic continuing medical education towards broader approaches grounded in andragogy, experiential learning, reflective practice, communities of practice and lifelong learning. These frameworks promote self-directed, practice-based learning, but CPD remains contested where activities are passive, attendance-based or reduced to regulatory compliance. Evidence suggests that knowledge acquisition alone rarely produces sustained behavioural change or clear improvements in patient outcomes. More effective CPD is active, contextualized, workplace-based, reflective and connected to feedback, quality improvement and outcome measurement. The review also considers innovations including digital learning, massive open online courses, spaced education, artificial intelligence, simulation, micro learning and just-in-time training, while recognising challenges such as cognitive overload, inequitable access, neurodiversity-related barriers and pharmaceutical industry influence. International approaches in the United Kingdom, Australia, Nigeria and the United States demonstrate a shared regulatory commitment to CPD but differ in emphasis. Overall, CPD is most valuable when it moves beyond checkbox compliance towards learner-centred, theory-informed and outcomes-focused professional development.
