Abstract

Multi-parametric magnetic resonance imaging (mpMRI) is a cornerstone for brain tumor diagnosis and treatment, yet current AI models face critical limitations: their lack of natural language interaction and interpretability impedes spatial information integration and cross-modal reasoning required clinically. Key challenges arise from significant physical meaning differences across modalities, spatial misalignment due to scan intervals, and the need for complex multi-feature interpretation in tasks like glioma grading. While visual-language models (VLMs) show promise in cross-modal understanding, existing methods focus mainly on 2D image modeling, neglecting direct perception of 3D volumetric space. Although 3D VLMs have been proposed for report generation and feature alignment in 3D CT imaging, mpMRI applications demand collaborative inference across multiple imaging modalities-a requirement unmet by current solutions. To address this, we introduce Mr3D-VL, a dedicated visual-language foundation model for multi-parametric 3D MRI. With 4 billion parameters, it employs an unsupervised pre-trained shared 3D encoder and 4D rotational positional embedding for dual modality-spatial integration. Its cross-modal projection layer uses a multi-resolution feature implantation strategy to enhance feature perception across resolutions. Experimental results show significant improvements over existing 4B/7B/30B domain-specific and general-purpose models in text generation tasks, achieving a BERTScore of 0.856 for report generation, with question-answering accuracy at 0.713 and multiple-choice accuracy at 0.912.

UniMedSeg: Unified In-Context Learning for Multi-Paradigm 2D/3D Medical Image Segmentation

Medical image segmentation foundation models are expected to generalize across diverse clinical scenarios, yet existing universal methods remain fragmented by prompt paradigms and spatial dimensions. Visual in-context learning, interactive segmentation, and language-guided segmentation are typically handled by paradigm-specific models, while 2D and 3D images are also modeled separately. Such isolation prevents heterogeneous annotations and data from being jointly absorbed by a single scalable model and limits cross-paradigm knowledge transfer. To address this bottleneck, we propose UniMedSeg, a Transformer-centric universal segmentation framework that maps visual examples, geometric interactions, language instructions, and 2D/3D images into a shared sequence space, enabling heterogeneous medical supervision to be jointly learned through a unified in-context interface without prompt- or dimension-specific branches. To overcome the long-sequence memory bottleneck caused by visual contexts, we introduce Decoupled Split Attention, which reduces attention complexity to linear while preserving hardware-friendly computation and focused context-target interaction. Extensively trained and evaluated on a large corpus curated from 27 public datasets, UniMedSeg achieves state-of-the-art performance across visual in-context, interactive, and language-guided segmentation without task-specific fine-tuning, demonstrating strong generalization on diverse held-out tasks. PerFact: Perception-Derived Fact Prompting for 3D Brain MRI Report Generation

Radiology report generation has matured almost entirely on 2D chest radiographs, where the default route to better reports is a larger backbone or a pre-training one on medical data. We revisit that assumption on 3D multi-sequence brain MRI, a volumetric multi-disease regime, and find that the model is not the lever. Zero-shot medical and radiology vision-language models transfer poorly to brain MRI, with chest radiograph specialists failing most conspicuously, and five backbones fine-tuned identically across three model families and an order of magnitude in scale differ only marginally. What determines the quality of the report is the information injected into the prompt. We delegate perception to upstream 3D segmentation and classification, serialize their outputs into a structured fact sentence, and prompt a LoRA-adapted vision-language model with it; we call this \textbf{PerFact}. In a controlled study that fixes the backbone, data split, target reports, and adaptation while varying only the injected grounding, perception-derived facts outperform retrieved prior reports, retrieval becomes redundant once facts are present, and end-to-end predicted facts remain effective without any ground-truth annotation at inference. The residual gap between predicted and oracle facts is explained by the granularity of the facts rather than by the generator. Closed-ended visual question answering comes at no measurable cost to report quality, though the grounding source has little effect on it. On 3D brain MRI, grounding information, not model choice, is the dominant controllable factor in report quality.

Multi-LLM Collaborative MRI Report Generation for Visual Instruction Tuning in Brain Oncology

Recent advances in large language models (LLMs) and their extension to vision-language models (VLMs) have made it easier to combine text and images for tasks such as report generation. Existing VLMs in medicine typically focus on 2D images (chest X-rays), and their extension to 3D imaging has been difficult because of the lack of paired 3D imaging-text data. Thus, we introduce a new method for creating a 3D image-text dataset for brain oncology using 3D MRI scans of glioma and meningioma cases. We use a cooperative system in which several LLMs work together to generate and check reports, ensuring that they are accurate and clear. By leveraging the new 3D MRI-text dataset, we further build a VLM that converts MRI scans into tokens and aligns them with text instructions. Our VLM performed better in report generation and visual question answering tasks than other 2D and 3D methods. Our method not only improves the quality of reports but also helps with better diagnosis and treatment in brain oncology.
