Abstract
Hereditary susceptibility accounts for a clinically meaningful proportion of epithelial ovarian, fallopian tube, and primary peritoneal cancers, with pathogenic variants in BRCA1 and BRCA2 producing the largest and best-characterized risks. This narrative and critical review examines how BRCA-associated risk should influence genetic identification, ovarian cancer surveillance, and preventive decision-making. Literature searches conducted in August 2026 prioritized clinical guidelines, systematic reviews, prospective cohorts, pivotal screening studies, and contemporary trials of risk-reducing surgery. Evidence consistently shows that BRCA1 and BRCA2 carriers have markedly different age-specific tubo-ovarian cancer risks, which supports gene-specific timing of risk-reducing salpingo-oophorectomy after completion of childbearing. Screening based on cancer antigen 125 and transvaginal ultrasonography may detect some cancers earlier, particularly when longitudinal algorithms are used, but it has not been shown to reduce mortality and should not be considered an equivalent alternative to preventive surgery. Salpingo-oo phorectomy substantially reduces tubo-ovarian cancer risk and is associated with improved survival, although premature menopause, quality-of-life effects, and residual primary peritoneal cancer risk require anticipatory management. Salpingectomy with delayed oophorectomy improves menopause-related outcomes in prospective studies, but its oncologic non-inferiority remains unproven. Effective prevention therefore depends on timely genetic testing, individualized counseling, technically complete surgery, and careful management of reproductive and menopausal consequences.
Individuals with pathogenic variants in BRCA1 and BRCA2 face increased risks of breast and ovarian cancer and are recommended to follow lifelong cancer prevention strategies to reduce their cancer-related morbidity and mortality. Many of these strategies, including annual imaging, risk-reducing surgery, and select medi...

Hereditary gynecologic cancer syndromes constitute a heterogeneous group of disorders in which pathogenic germline variants increase susceptibility to ovarian, fallopian tube, endometrial, and other related malignancies. This study analyzed current evidence regarding the clinical identification of individuals at increa...

Moderate-risk gene variants represent an increasingly recognized component of hereditary breast and ovarian cancer (HBOC) predisposition, yet their clinical interpretation and management remain less standardized than those of high-penetrance genes. This study provides a seven-year real-world evaluation of genetic varia...

BRCA1/2 mutation status is a key biomarker in the management of ovarian cancer, supporting personalized treatment strategies and improving patient outcomes, and emerging mechanisms of resistance to PARP inhibitors remain a significant therapeutic challenge.

Breast cancer risk after ovarian cancer in germline BRCA1/2 heterozygotes remains uncertain, with a recent large multicentre international study reporting lower-than-expected incidence in the first decade after ovarian cancer diagnosis (Apostol et al. 2026). We re‑evaluated our previously published regional cohort of 7...

Abstract Women with BRCA1 pathogenic variants face high lifetime risks of breast and ovarian cancer, requiring complex risk-reducing decisions. We report a 35-year-old woman with node-negative triple-negative breast cancer who achieved a pathologic complete response after neoadjuvant chemotherapy and breast-conserving...
