Gut microbiome and metabolite signatures in critical limb threatening ischemia.

Background: Atherosclerotic disease entails with distinct clinical phenotypes, of which chronic coronary syndrome (CCS) and critical limb-threatening ischemia (CLTI) are key manifestations. While plaque morphology and clinical course differ in vascular territory, the underlying pathophysiology leading to distinct disea...

Programmed Cell Death-1/Programmed Cell Death-Ligand 1 Pathway in High-Risk Lymphocytic Acute Myocarditis

Background The programmed cell death-1 (PD-1)/programmed cell death-ligand-1 (PD-L1) pathway is affected in immune checkpoint inhibitor (ICI)-associated acute myocarditis (AM), while this pathway in non-ICI-associated lymphocytic AM (LAM) is unexplored. Objectives This study aimed to assess cardiac levels of PD-L1/PD-1...

Tricuspid-Valve Intervention in Heart Failure.

BACKGROUND The effect of transcatheter tricuspid-valve repair on clinical outcomes, including death and hospitalization for heart failure, in patients with severe tricuspid regurgitation remains uncertain. METHODS We randomly assigned patients with symptomatic severe tricuspid regurgitation and an increased risk of f...

Three-dimensional mitral valve geometry after transcatheter edge-to-edge repair: PASCAL versus PASCAL-ACE-implications for devices selection.

BACKGROUND The PASCAL® transcatheter edge-to-edge repair system (TEER) is established for mitral regurgitation (MR), with the ACE iteration tailored for complex anatomies. Comparative data are limited. AIMS To compare 3D mitral valve remodeling and procedural outcomes in patients undergoing TEER with PASCAL or PASCAL...

Dual antithrombotic therapy using potent antiplatelet inhibitors in atrial fibrillation and acute coronary syndrome: a randomized controlled trial.

Treatment with a potent P2Y12 inhibitor was associated with higher bleeding rates without a clear reduction in the risk of ischemic complications, and these findings do not support the routine use of potent P2Y12 inhibitors in combination with DOACs in this patient population.

Prognostic Value of NYHA Functional Class in Tricuspid Valve Transcatheter Edge-to-Edge Repair: Insights From EuroTR.

BACKGROUND In patients undergoing tricuspid valve transcatheter edge-to-edge repair (T-TEER) for tricuspid regurgitation (TR), NYHA functional class guides assessment, but real-world data on its prognostic value and determinants of symptomatic response remain limited. OBJECTIVES The objective of the study was to asse...

One-Year Outcomes of M-TEER With the PASCAL System in Atrial and Ventricular FMR: Insights From the MiCLASP Postmarket Clinical Follow-Up Study.

One-year MiCLASP results support the safety of mitral transcatheter edge-to-edge repair with the PASCAL system in both AFMR and VFMR, with comparable echocardiographic, functional, and quality-of-life changes, despite baseline differences in clinical presentation.

An explainable transformer model learning from entire treatment timelines for pan-cancer risk profiling across healthcare systems

Chronicle is introduced, an explainable transformer that learns from entire patient trajectories, predicts diverse clinical outcomes throughout the disease course while capturing both short- and long-term temporal dependencies, providing a scalable framework to support individualized treatment decisions.
